Curvatures
Penile curvature is defined as angulations observed either from birth (congenital curves) or at some point in a man’s life, usually after a penile injury (Peyronie’s disease).
Congenital Curvature
Congenital curvatures result from malformations during child development, where the penis does not grow symmetrically. Usually, the lower surface or urethra is underdeveloped, resulting in a downward angulation of the penis. Surgical restoration is the only treatment option.
It is crucial to address congenital bends during the preadolescent period (before the age of 10) to prevent significant psychological effects. The problem with congenital curvature is that the angulation and turning of the penis are not visible when it is flaccid.
Consequently, many children keep the issue a secret out of shame and reluctance to share it with their parents. This secrecy can lead to delayed treatment and avoidance of initiating sexual activity until later in life.

The condition is also associated with another rare disease, Dupuytren’s disease, characterized by a constant inward curvature of one or more fingers of the hand due to the growth of fibrous tissue between the fingers and palm.
Penile Fracture
Peyronie’s disease is often associated with traumatic injury during sexual activity. Some men recall the moment of injury, describing it as momentary pain or a “crack” upon contact. However, many do not remember any injury due to the intensity of the sexual encounter. In younger individuals, injury may be associated with specific sexual positions (e.g., partner on top), while in older individuals, reduced erectile function and vaginal dryness can contribute decisively, such as in cases of erectile dysfunction and vaginal atrophy during menopause.
Penile fracture is a distinct condition from Peyronie’s disease, involving the complete rupture of the fibrous membrane (tunica albuginea) surrounding the two corpora cavernosa of the penis. It is accompanied by extensive hematoma, pain, and significant swelling, resulting in deformation of the penis.
Penile fracture requires immediate surgical treatment by an experienced surgeon. In rare diseases, such as penile fracture, limited cases make it essential to rely on expertise from reference physicians specializing in the condition.
